Why is treatment done earlier than admission?

Sometimes, when a case is urgent and vital, hospitals may proceed with delivering medical care right away to save a life, even before the finalization of admission procedures. Usually, when this happens, hospitals put the bill on our credit until the funds are transferred to their accounts.

That's why the chronology can sometimes be in disorder.

Background

Anvy is a 19-year-old young woman living in a rural setting, where she has taken on significant responsibility to support her family. She runs a small business earning about KSh 1,000, which contributes to meeting basic household needs. Despite financial challenges, she remains resilient, caring, and determined to improve her family’s living conditions. She presented to a nearby health facility with lower abdominal pain consistent with labour and was admitted for monitoring. Although labour initially progressed normally, it became prolonged, and signs of fetal distress developed, raising concern for both her and the baby. She was advised to seek urgent care at a higher-level facility for a caesarean section but expressed concern about her inability to afford the procedure. After assessment of her socioeconomic status, she was referred to St. Elizabeth Hospital Mukumu, where support for underprivileged patients is available. On arrival, Anvy was exhausted and in distress, with severe abdominal pain, dehydration, and tachycardia. Fetal assessment confirmed ongoing distress, necessitating immediate preparation for emergency obstetric intervention.

Medical history

Anvy developed complications during labor and was urgently taken for a Cesarean Section to ensure the safety of both mother and baby. With support from Helpster Charity Organization, she was able to receive the surgery and necessary hospital care at no cost. After the operation, she was closely monitored, given pain management, antibiotics to prevent infection, and proper postnatal care to support healing.Following treatment, Anvy recovered well, with her condition stabilizing and the surgical wound healing properly. Both mother and baby were discharged in good health.

Prognosis

Anvy presented with prolonged labour complicated by signs of fetal distress and maternal exhaustion. The decision to perform an emergency caesarean section was appropriate to prevent further complications and safeguard both mother and baby. She has been prepared for surgery and is under close medical care. With timely intervention, the prognosis for both Anvy and the unborn baby is good. The procedure is expected to relieve the obstruction and allow safe delivery. However, without prompt surgical management, there would have been significant risks, including fetal compromise, maternal complications, or loss of life.
