Why is treatment done earlier than admission?

Sometimes, when a case is urgent and vital, hospitals may proceed with delivering medical care right away to save a life, even before the finalization of admission procedures. Usually, when this happens, hospitals put the bill on our credit until the funds are transferred to their accounts.

That's why the chronology can sometimes be in disorder.

Background

Era Manager is a 21-year-old single woman who is jovial and enjoys travelling, singing, and household activities. She lives with her uncle, aunt, and their seven children in a three-bedroom house. Her uncle previously supported her education through secondary school but lost his job, making it impossible for her to continue. Era dreams of studying Business Administration and becoming a successful businesswoman but currently has no means to pursue her education or start a small business. Era was brought to the facility by relatives in an unconscious state after three days of severe one-sided headache, dizziness, high-grade fever of 39.5°C, poor appetite, and refusal to eat. Her condition progressively worsened before she lost consciousness. On arrival, she was critically ill and dehydrated. Immediate resuscitation and intravenous fluids were administered. Evaluation at our facility confirmed the diagnosis of Severe Malaria, Typhoid, Anemia, and dehydration. Era was admitted for urgent medical care.

Medical history

Following resuscitation and aggressive rehydration therapy, the patient showed progressive improvement in her level of consciousness and general clinical condition. Her hydration status improved significantly, and she gradually regained responsiveness. The severe headache and dizziness reduced in intensity, while her body temperature responded favorably to treatment. As recovery progressed, Era was able to tolerate oral fluids and subsequently resumed feeding. Continuous monitoring confirmed stable vital signs and satisfactory clinical improvement. She is currently conscious, clinically stable, and responding well to treatment. Further evaluation and follow-up management have been recommended to identify and address the underlying cause of her illness and to ensure complete recovery.

Prognosis

Doctor's Opinion Era was brought to the facility in an unconscious state following a three-day history of severe unilateral headache, dizziness, high-grade fever (39.5°C), poor oral intake, and refusal of meals. Clinical assessment revealed significant dehydration and an altered level of consciousness, suggestive of a severe systemic illness requiring urgent medical intervention. The prognosis is currently guarded but favorable, given the patient's positive response to resuscitation and rehydration measures. With continued monitoring, appropriate diagnostic evaluation, and timely treatment of the underlying condition, a good recovery is anticipated. However, close observation remains necessary to detect and manage any potential complications that may arise.
