Why is treatment done earlier than admission?
Sometimes, when a case is urgent and vital, hospitals may proceed with delivering medical care right away to save a life, even before the finalization of admission procedures. Usually, when this happens, hospitals put the bill on our credit until the funds are transferred to their accounts.
That's why the chronology can sometimes be in disorder.

Background
Brilian is an 18-year-old young woman who is respectful, responsible and kind. She enjoys singing and dreams of becoming a successful musician. She lives with her mother in a remote rural area, where the household depends on her mother’s irregular casual labour. The modest income is often barely sufficient for their daily needs, making unexpected healthcare expenses difficult to afford. Brilian was brought to hospital in labour by her relatives after developing severe lower abdominal pain radiating to the back, accompanied by headache. She underwent clinical examination, laboratory tests and an ultrasound. During labour, cervical dilatation remained at about 5 cm for a prolonged period, while concerns were also noted regarding the fetal heart rate and the baby’s position in the pelvis. Further assessment revealed a kneeling breech presentation, an abnormal presentation in which the baby’s knees are positioned to come first through the birth canal. Given the failure of labour to progress, abnormal presentation and concern for fetal wellbeing, the medical team recommended an emergency Caesarean section.
Medical history
Brilian brought to the facility in labour with history of; severe abdominal pain radiating to the back. On examination, laboratory and ultrasound, she was at 5cm dilated for long period without progressing, abnormal fetal heart rate. Based on findings, she was diagnosed with kneeling breech presentation. Due to this complications medical team recommended for emergency ceaserian section as the safety method and to high risk to occur. The operation was successful both mother and the baby were doing well. Was given: I.v Normal saline, I.v Ceftroxane, I.v paracetamol, I.v Metronidazole and I.v 5%D. She responded positively to medication and discharged home when stable and in good condition.
Prognosis
Brilian, an 18-year-old woman, has a kneeling breech presentation, which is a very rare type of breech presentation where one or both knees present first. This is considered a high-risk situation because it increases the risk of umbilical cord prolapse, difficult delivery, and injury to the baby. In most cases, a caesarean section (C-section) is recommended to improve the safety of both the mother and the baby. With prompt diagnosis, close monitoring, and delivery in a well-equipped health facility, the prognosis is generally good for both Brilian and her baby.
