Timothy is in Grade I and would like to be a doctor when he grows up. He lives with his brother Simiyu who is also unwell, together with their 2 sisters and parents. Both of his parents do not have any jobs. His father does manual jobs to earn a living. They get their water from a stream which takes more than 40 mins to go to the stream and back to the homestead. They use their neighbour’s toilet since their homestead does not have a toilet. Their house is grass thatched and has mud walls. The floor is earth and his mother covers it with dung once a month. They do not have electricity in the homestead. He and his brother have been unwell, passing dark urine and having pain in the flank regions. When he grows up, he would like to be a pilot. Timothy has been treated as an in-patient for UTI and nephritis. Laboratory tests were done to confirm the diagnosis. He responded well to treatment and was discharged to proceed with oral medication at home.

The patient was presented to Bokoli Subcounty Hospital with hotness of body, pain on urination, dark urine, fever, and vomiting. The patient is diagnosed with nephritis, due to urinary tract infection. If not treated, the symptoms will persist and worsen, leading to a soon kidney failure.
