Why is treatment done earlier than admission?

Background

Samuel is a 15 y.o. boy who lives in one bedroom apartment together with 5 members of his family. The mother is a petty trader while the father is a farmer. His father doesn’t live with the family. The estimated income of the mother is 15000 naira per month. The lesion appeared like a boil on the right hand in 2022. He was taken to the church where they applied local concoction on the lesion. The lesion became septic and the skin became necrotized.

Medical history

Samuel was treated for Necrotising fascitis, a flesh-eating disease. He underwent a procedure called skin grafting, his wounds were cleaned, and he was discharged on the 4th of September.

Outcome

The patient was presented to Divine Amazing Grace Hospital with the extensive skin loss on the side of the chest and posteriorly. The patient needs a septic work up, blood transfusion and a surgery. If not treated soon, the condition may lead to bleeding, sepsis, and further necrotizing.
