Melvin A., 17

Less than two years after giving birth to her first child, Melvin discovered she was pregnant again - already four months along. Together with her husband, they visited the nearest dispensary, where she began her antenatal care and managed to attend four clinic visits. She was referred for an ultrasound, but was unable to go due to financial constraints. One day, while fetching water from a well, Melvin experienced sudden pain as she lifted jerrycans onto her head. A woman who was with her quickly contacted Melvin’s husband, who came to help her and carry the water. She later presented at the health facility with abdominal pain radiating to her back, which had started about three hours earlier. On examination, she was found to be in the active phase of labor: the abdomen was gravid, fundal height 38 weeks, fetal lie was left occipito-anterior (LOA), and descent was 2/5. She was admitted for close monitoring and possible vaginal delivery. Melvin is a young mother who was married off after becoming pregnant with her first child. She is a housewife who relies entirely on her husband, a motorbike rider who earns just enough to get by. Despite her challenges, Melvin is an active choir member in her local church and dreams of becoming a gospel singer one day.

Melvin reported to the facility with a history of abdominal pain radiating to back with the onset of 3 hours. On examination the abdomen was gravid, FH 38/40, Lie - LOA, Descent 2/5 in. She was already in the active phase of and was admitted for monitoring of the progress of labor and possible vaginal delivery. At 1240 hours, she gave birth to a bouncing baby girl who scored well as per the APGAR score. She was duly discharged and advised to visit the hospital for her postnatal clinics.

The prognosis for normal delivery, specifically spontaneous vaginal delivery in women with uncomplicated term pregnancies, is generally good. Most women experience a successful vaginal delivery, with very few adverse neonatal outcomes. After delivery, both mother and infant are typically in good condition.
