Why is treatment done earlier than admission?

Sometimes, when a case is urgent and vital, hospitals may proceed with delivering medical care right away to save a life, even before the finalization of admission procedures. Usually, when this happens, hospitals put the bill on our credit until the funds are transferred to their accounts.
That's why the chronology can sometimes be in disorder.

Background

Ayaz was born with intellectual disabilities and relies entirely on her elderly mother for daily care and support. They live in a remote rural area, less than a kilometer from the border, where they use untreated river water for drinking and household needs and share toilet facilities with neighbors. The family lives in extremely vulnerable conditions. She was referred from a peripheral health facility with a labour pains, history of lower abdominal pain radiating to the back, which had become more frequent and intense. At the referring center, labor had progressed to full cervical dilation, but there was poor descent of the fetal head. On vaginal examination, the cervix was fully dilated, there was significant caput formation, and meconium-stained amniotic fluid was detected, indicating fetal distress. The diagnosis was obstructed labor with meconium-stained liquor Grade III in a primigravida at term, likely due to cephalopelvic disproportion. An emergency caesarean section was planned and performed, resulting in the delivery of a live male infant. Due to her mental health condition, the patient was unable to comply with post-operative care instructions. Despite the efforts of her elderly mother and the nursing staff, adherence to wound care and medication was challenging. By the fifth post-operative day, she developed bilateral lower limb swelling, and investigations revealed hypoalbuminaemia. She also had abdominal distension, significant pain at the surgical site, and foul-smelling discharge from the incision. Examination confirmed wound dehiscence and seroma formation. She was pale, and further tests showed anemia, for which she received a blood transfusion. She underwent a secondary wound closure in the operating theater and, during her recovery, developed intestinal obstruction, which was successfully managed conservatively without further surgery. Her hospital stay lasted 52 days, during which she gradually improved. She was discharged in stable condition, and her baby remained well throughout her prolonged admission.

Outcome

Obstructed labor is an obstetric emergency and the need for an emergency caesarean section was a life saving intervention to both the mother and the child. It was because of the interventions made that we had the best outcome to this young mother Post-operative care after abdominal and uterine surgery is crucial for proper healing, preventing infections, and avoiding complications such as bleeding, wound dehiscence, or organ damage. It ensures the surgical site remains clean, promotes early detection of problems like infections or clots, supports pain management, and helps the patient regain strength and mobility for a full recovery.
