Why is treatment done earlier than admission?

Sometimes, when a case is urgent and vital, hospitals may proceed with delivering medical care right away to save a life, even before the finalization of admission procedures. Usually, when this happens, hospitals put the bill on our credit until the funds are transferred to their accounts.
That's why the chronology can sometimes be in disorder.

Background

Shinky is the only child of her parents and lives with them in a small single-room rental house. Although the initial rent was Ksh 2,000, the landlord, moved by their difficult circumstances, kindly allowed them to stay without paying rent as his way of supporting the family. Shinky’s mother is currently unemployed, leaving the father as the sole provider. He does his best to support the family by taking on casual labour and small jobs. Shinky’s mother recalls that a few months after her birth, she became worried about her baby’s development. At around four months, she noticed that Shinky wasn’t growing as expected. When she tried to support her in a sitting position using a basin, Shinky couldn’t hold her head up and would slump forward. Attempts to help her stand were also unsuccessful, as her legs could not support her weight. In addition, Shinky was not breastfeeding well. Concerned about her daughter’s health and development, the mother sought medical attention at the hospital. Sinky was diagnosed with severe pneumonia, hydrocephalus and heart failure.

Medical history

Shinky was received in the emergency department, experiencing difficulty in breathing, coughing, and an inability to sit, and was sick-looking. Her bloodwork was sent to the lab, and other examinations were done. She was diagnosed with heart failure and severe pneumonia; these warranted admission to the ward. Treatment was done as per the t-sheet, to which she was tolerant. Eventually, Shinky was discharged with a follow up set for two weeks after her discharge. Her mother was also advised to visit Jaramogi Oginga Odinga Teaching and Referral hospital for surgery, as Siaya County Referral Hospital was not equipped to handle such a surgery

Outcome

The prognosis of an infant with severe pneumonia, heart failure, and hydrocephalus depends on several factors including the severity of each condition, timing and quality of treatment, the underlying cause, and the infant’s overall health and nutrition status. When all three conditions are present in an infant, the risk is very high, and prognosis quite poor, especially if access to advanced medical care is limited. Heart failure prognosis varies significantly based on several factors, including the underlying cause, severity, and individual response to treatment. Generally, one-year survival rates for chronic heart failure range from 80-90%, while five-year survival rates fall between 50-60%.
