Abdulkarim M., 7

Abdulkarim is a 7-year-old boy living with his parents in a low-income household with limited access to basic amenities, including electricity and clean water. His father is a petty trader and his mother a housewife, and the family struggles to meet daily needs. He attends a government primary school despite these challenges. He developed bilateral neck swelling over four days, which progressively increased despite initial home treatment, along with fever, cough, and ear pain. Due to financial constraints, his parents were unable to seek timely hospital care. On examination, he appeared ill, febrile, mildly dyspneic, and pale, with tender swelling around both parotid regions extending to the upper neck. He was diagnosed with parotitis and a severe lower respiratory infection, requiring urgent medical attention and supportive care.

Abdulkarim's illness started 4 days ago with neck swelling bilaterally which increased in size despite taking ampiclox and brufen for 3 days, other accompanying complaints are fever, cough, and ear pain. On examination a 7yo male child, ill looking, febrile (37.5o C), mildly dyspneic, unicteric, acyanosed, palor. He was diagnosed with Parotitis and severe lower respiratory infection. Treatment commenced with injection antibiotics, antipyretics, some cough syrup and other supportive care. Patient did well on the medication, he became stable with temperature dropping back to normal, swelling beneath jaw was gone. He was discharged, with oral medications to return after a week for follow. The family are happy and are grateful to Helpster Charity for the medical support.

Parotitis is an inflammation of the parotid glands, often associated with infection, and can present with painful swelling around the jaw and neck. When it occurs alongside a severe lower respiratory tract infection, it indicates a significant systemic illness that may compromise breathing and overall health, particularly in children, and requires prompt medical treatment. With appropriate management, including antibiotics, supportive care, and close monitoring, recovery is achievable. Prognosis is generally good with timely intervention, but delayed or inadequate treatment may lead to complications such as abscess formation, worsening respiratory distress, or systemic infection.
