Ramlat is a 5 year old female child living with her parents in a small community outside the city of Bauchi. She is the 4th child among 5 siblings. She is not in any school due to unavailability of school in their community. The father is a laborer and the mother is a house wife. They mostly struggle before they can feed. They live in a 3 rooms apartment having a pit toilet. They make use of water from the borehole for daily activities. They don't have access to electricity. Ramlat's illness started a week ago with a productive cough of whitish sputum, associated with chest pain and she developed high grade fever 4 days ago with abdominal pain. Mother noticed bilateral subauricular painful swelling. On examination, a 5year old female child, lethargic, febrile (38.7o C), anicteric, acyanosed and mildly pale with bilateral subauricular tenderness. Inter and subcostal swelling noticed. Family couldn't find the money for the medical bills, they contacted a volunteer and they were directed to come to the hospital for proper evaluation. She was diagnosed with Severe Malaria, Parotitis and Anemia.

She was diagnosed with Severe Malaria, Parotitis and Anemia. Treatment commenced with injection antimalarials, injection antibiotics, antipyretics to bring down the temperature, some cough syrup and vitamins as well as pain relief. She was transfused with a pack of red blood cell. Patient became stable after hospital stay, temperature was back to normal, cough stopped and eventually patient was discharged with some oral medications, she is to be returned for follow up after 1 week. Management involves prompt administration of antimalarial therapy, treatment of the underlying infection causing parotitis with appropriate antibiotics if bacterial, adequate hydration, antipyretics, and correction of anemia, which may require blood transfusion in severe cases. Close monitoring is essential to prevent complications and ensure recovery. The prognosis of severe anemia combined with parotitis is good but also depends entirely on the underlying systemic disease causing both conditions, such as severe infection, autoimmune disorders, or malignancy which when managed will make the patient feels better.
