Why is treatment done earlier than admission?

Sometimes, when a case is urgent and vital, hospitals may proceed with delivering medical care right away to save a life, even before the finalization of admission procedures. Usually, when this happens, hospitals put the bill on our credit until the funds are transferred to their accounts.
That's why the chronology can sometimes be in disorder.

Background

Yusuf is a 2 year old boy currently staying with his parents in a polygamous setting. The father is a laborer, the mother is a house wife, they stay in a 2 rooms apartment, they barely feed twice a day. They make use of water from the well, there is a pit toilet for the whole members of the family. They have no electricity in the community. Presenting complain: Fever, facial swelling, upper and lower swelling, crying excessively, catarrh, runny nose and poor appetite. For more than one week, Yusuf has been having fever which happened to be on/off. Three days ago, mother noticed facial swelling, upper and lower limbs swelling associated with paleness. His nose was runny, with cough, he cries on/off as the fever runs intermittently. On examination, very ill looking pale but not icteric or acyanosed, palor++ and very febrile (39.5o C),and tachypneic. Diagnosis of severe Malaria, Anemia & Enteritis was made. The family are poor and could not take him to the hospital until they got information about Helpster Charity in their community. The child was brought to our facility, evaluated thoroughly and admitted to urgent medical treatment.

Medical history

For more than one week, Yusuf has been having fever which happened to be on/off. Three days ago, mother noticed facial swelling, upper and lower limbs swelling associated with paleness. His nose was runny, with cough, he cries on/off as the fever runs intermittently. On examination, very ill looking pale but not icteric or acyanosed. Palor++ and very febrile (39.5o C) Tachypneic Diagnosis of severe Malaria, Anemia & Enteritis was made. Yusuf was admitted and was managed with injection antimalarial, injection antibiotics, blood was transfused and other supportive care as well as close monitoring until patient was stable. Fever stopped, swelling was gone, crying. Patient is well and was discharged with some oral medications. He is to return for follow up after 1 week. The family extends gratitude to Helpster Charity and the entire team.

Prognosis

Palor++ and very febrile (39.5o C) Tachypneic Diagnosis of severe Malaria, Anemia & Enteritis was made. Management includes antimalarial therapy, rehydration, correction of anemia, and supportive care to prevent complications.
