Why is treatment done earlier than admission?

Sometimes, when a case is urgent and vital, hospitals may proceed with delivering medical care right away to save a life, even before the finalization of admission procedures. Usually, when this happens, hospitals put the bill on our credit until the funds are transferred to their accounts.

Background

Usman is a 12-year old, who comes from a low-income family earning less than ₦20,000 per month. The household lacks electricity and running water, relying on a community borehole, which may contribute to hygiene challenges and health risks. Despite these conditions, he remains active, helps with household chores, and attends school when possible. He enjoys playing football and drawing and aspires to become a doctor in the future to support his family and contribute to his community. Usman Abubakar, an 11-year-old boy, developed symptoms about one week before presentation. The illness started gradually with pain during urination (dysuria), followed by increased frequency of urination. A few days later, he began to notice occasional blood in his urine (hematuria). The likely cause of his condition is the formation of a bladder stone (cystolithiasis). This may have developed over time, possibly related to factors such as low fluid intake, dehydration, or urinary stasis—conditions that can be more common in settings with limited access to clean water. At the time of evaluation, Usman is in a stable condition. He is active and playful, with normal vital signs. Physical examination shows only mild tenderness in the lower abdomen (suprapubic region), with no severe distress or complications. Investigations confirmed the presence of a fairly large bladder stone, which explains his symptoms. Currently, he is diagnosed with cystolithiasis and is planned for surgical removal of the stone (cystolithotomy). His overall condition is stable, and with appropriate treatment, the prognosis is good.

Medical history

Usman, a 12-year-old boy, was admitted with a bladder stone and underwent a successful cystolithotomy (surgical removal of the bladder stone). He received appropriate preoperative and postoperative care, medications, and close monitoring. Usman recovered well after surgery. His symptoms resolved, he was able to urinate normally, and he was discharged home in good condition, ready to resume his normal daily activities. Prognosis

In my opinion, Usman has a stone of 3.5 cm is too big for a child to pas via urine. Due to pains and discomfort and prevent complications such as infections or kidney injury. Usman needs Cystolithotomy, to prevent further danger and complications. PROGNOSIS; Good prognosis with prompt diagnosis and timely treatment. Long term prognosis is excellent if the cause is found and treated. The key in this case is the immediate removal of the stone now and figuring out why it formed and preventing recurrence.
