Background

Darwin is a cheerful and energetic 3-year-old boy who enjoys playing with other children and exploring his surroundings. He lives with his parents in a rural village where the family faces considerable financial hardship. His parents rely on casual labor jobs that provide irregular and limited income, making it difficult to meet basic needs and access healthcare when illness occurs. Despite these challenges, Darwin is raised in a loving and supportive home, and his parents remain hopeful for a better future for their son. Darwin was brought to the hospital by his grandmother after experiencing several months of persistent loud snoring, mouth breathing, restless sleep, and frequent night awakenings. She also reported episodes of choking and brief pauses in breathing during sleep, as well as poor feeding, daytime fatigue, and irritability. He had a history of recurrent upper respiratory tract infections and persistent nasal congestion. On examination, Darwin was breathing through his mouth and was found to have markedly enlarged tonsils that were nearly meeting at the midline, together with enlarged adenoids causing significant narrowing of the airway. Based on these findings, he was diagnosed with severe tonsillar hypertrophy and adenoidal enlargement leading to upper airway obstruction. To relieve the obstruction and prevent further complications, he was scheduled for an adenotonsillectomy under general anesthesia.

Medical history

Darwin Kelly is a 3-year-old boy who was brought to our facility by his grandmother with a history suggestive of upper airway obstruction. He had been experiencing persistent loud snoring, mouth breathing, and restless sleep for several months. Based on the clinical presentation and examination findings, a diagnosis of severe tonsillar hypertrophy with adenoidal enlargement causing upper airway obstruction was made. Darwin Kelly subsequently underwent an adenotonsillectomy under general anesthesia. The procedure was completed successfully without intraoperative complications. He discharged home when stabilized and in good condition he went home happily.

Outcome

Hypertrophy of the adenoids and tonsils that warrants surgery generally has an excellent prognosis. Following adenoidectomy, tonsillectomy, or both, most patients experience significant improvement or complete resolution of airway obstruction, snoring, sleep-disordered breathing, recurrent infections, and swallowing difficulties. Recovery is usually complete within 1–2 weeks, and long-term outcomes are favorable with a marked improvement in quality of life. Although postoperative pain and a small risk of bleeding are expected considerations, serious complications are uncommon. Recurrence of symptoms is rare, particularly when surgery is performed for appropriate indications, though persistent symptoms may occur in patients with underlying conditions such as obesity, allergies, or craniofacial abnormalities. Overall, the prognosis is excellent with timely surgical intervention and appropriate postoperative care. Darwin's prognosis is good
