Javan J., 4

Javan is a four-year-old boy who lives with both his parents and his twin sister, whom he loves dearly. He is in kindergarten and enjoys singing and playing with his sister. His mother is the family’s breadwinner and survives through small-scale hawking of small dried fish to support her children with the little income she earns daily. Javan was brought to the facility in a critically ill condition requiring urgent medical intervention. He had been referred from Awasi Mission Hospital with a history of fatigue, poor feeding, intermittent fever, and scrotal swelling. According to his caregivers, he had become progressively weak and less active, raising concern about his health. On examination, he appeared severely weak and pale. An inguino-scrotal ultrasound was performed and suggested bilateral hydrocele. Further assessment confirmed a diagnosis of severe anaemia and bilateral hydrocele. Due to the severity of his condition, the doctor recommended urgent transfusion of two units of blood to stabilize his haemoglobin levels before surgery. Surgical correction was later advised to prevent future hernia formation and possible complications.

The patient presented to the facility with history of fatigue, poor feeding and intermittent fever with associated scrotal swelling. A diagnosis of severe anaemia and bilateral hydrocele was made. He was transfused two units of blood and underwent a succesful surgery. He was then discharged while in a stable condition with the discharge drugs as per the attached dsicharge summary. The patient has been advised to return for further review if symptoms persist before the indicated review date.
