Why is treatment done earlier than admission?

Sometimes, when a case is urgent and vital, hospitals may proceed with delivering medical care right away to save a life, even before the finalization of admission procedures. Usually, when this happens, hospitals put the bill on our credit until the funds are transferred to their accounts.
That's why the chronology can sometimes be in disorder.

Background

Tobias is the firstborn in a family of four children and is currently in Grade 9. He is vibrant, active, and enjoys playing football. He dreams of becoming an engineer in the future. Tobias lives with both parents in a rural setting under difficult financial conditions. His father is a small-scale farmer who grows vegetables to feed the family and pay school fees, while his mother is a housewife. The family struggles to meet basic needs and often faces challenges accessing proper healthcare due to limited income. Tobias remained healthy until 13th April when he developed stomachache and headache. His condition progressively worsened, with persistent fever, weakness, poor appetite, chest congestion, body malaise, and difficulty carrying out normal activities. His father initially sought treatment from a nearby chemist, where he was given AL, paracetamol, and salbutamol, but there was no improvement. The following day, Tobias became more unwell, prompting his mother to take him to the hospital. After clinical assessment and laboratory investigations, he was diagnosed with severe pneumonia, gastroenteritis, and severe malaria.

Medical history

Tobias came to the hospital when he had been given some medications the father bought for him but with no improvement. He was assessed and samples taken for investigation and diagnoses of Pneumonia, malaria and gastroenteritis was made. He was commenced on IV fluid RL/D5%,Ceftriaxone,Flagyl,and Artesunate. Upon day 3 post admission he was stable and the mother requested for discharge however the Clinical officer advised for extension of one more day to get the antibiotic then later discharged on oral medications. He has fully recovered and has gone back to school and can also help in house chores.

Outcome

Tobias presented in a significantly unwell condition with persistent fever, weakness, chest congestion, stomach pain, poor appetite, and body malaise. Clinical examination and laboratory investigations confirmed severe pneumonia, gastroenteritis, and severe malaria, all of which required urgent medical treatment and close monitoring. Delayed access to proper healthcare due to financial challenges contributed to worsening of his condition before admission. He was started on appropriate medication, supportive therapy, hydration, and has a good prognosis.
