Background

Sahura Dayyabu is a 20-year-old single mother of two, residing in Garin Lamido. She has been diagnosed with upper gastrointestinal bleeding secondary to peptic ulcer disease (PUD). She works as a cleaner, earning a monthly income of ₦10,000, which is insufficient to meet her basic needs and medical expenses. Sahura lives with her children in a mud house. She was well until about one month ago when she developed a burning upper abdominal pain that progressively worsened. A week before presentation, she began passing black stools, followed by vomiting, which later contained dark blood. Her condition was accompanied by weakness, dizziness, and breathing difficulty. She is currently in a serious condition, diagnosed with upper gastrointestinal bleeding secondary to peptic ulcer disease, and requires urgent medical care.

Medical history

Sahura was stabilized with intravenous fluids and medications to reduce stomach acid and control the bleeding. A blood transfusion was administered to correct blood loss, and she was closely monitored throughout her care. Further evaluation, including endoscopy, was performed to identify and manage the source of bleeding. She was also started on medications to treat the underlying ulcer and help prevent recurrence. Outcome: She has responded well to treatment, with significant reduction in symptoms and stabilization of her condition. With continued medication, proper follow-up, and ongoing care, she is expected to make a full recovery.

Prognosis

The patient is a 20-year-old female diagnosed with upper gastrointestinal bleeding secondary to peptic ulcer disease. Her condition is serious with risk of hypovolemic shock from massive blood loss, severe anemia, and gastrointestinal perforation, which can rapidly cause multi-organ failure or death. She requires urgent stabilization and further evaluation such as endoscopy to identify and control the source of bleeding. Prognosis: With early intervention and proper treatment, her prognosis is fair to good, and she is expected to recover. However, delay in treatment or lack of adequate care may lead to complications such as severe anemia, recurrent bleeding, or shock, which can be life-threatening.
