Rumaisau is a 19yo female child who just finished her secondary education. She wishes to study nursing but her concern is about her sponsor, the parents are poor. She stays with her parents and siblings in an inherited apartment having 3 rooms, pit toilet, a well, no access electricity. They have 15,000 monthly income. Rumaisau’s illness started a week ago with headache, lower abdominal pain, back pain and fever. She became dizzy, two days later associated with poor appetite and vomiting everything ingested. In addition to above she also has pelvic discharge. On examination, a 19year old lady, weak, ill looking, febrile (37.8o C), unicteric, acyanosed, no palor. She was diagnosed with acute pelvic inflammatory disease and severe malaria. She couldn't afford the bills for medication that is why she contacted Helpster Charity for the medical support.

Rumaisau’s illness started a week ago with headache, lower abdominal pain, backpain an fever. She became dizzy, two days later associated with poor appetite and vomiting everything ingested. In addition to above she also has pelvic discharge. On examination, a 19year old lady, weak, ill looking, febrile (37.8o C), unicteric, acyanosed, no palor. She was diagnosed with acute pelvic inflammatory disease and severe malaria. She was admitted and treatment commenced with injection antimalarial, injection antibiotics and other supportive care such as IV fluids and some antipyretics, she responded well to treatment until stable and discharged. She is to come for follow up visit in a week time. She extend her gratitude to Helpster charity and the entire team for the medical support.

Rumaisau was diagnosed with severe malaria requiring urgent hospital-based management due to the risk of complications such as severe anemia, cerebral involvement, hypoglycemia, and multi-organ dysfunction. Prognosis is good with early diagnosis, prompt administration of intravenous antimalarial medications, adequate hydration, and close monitoring of vital parameters and laboratory indices. The clinical features are also suggestive of acute pelvic inflammatory disease, likely secondary to ascending genital tract infection. Early initiation of broad-spectrum antibiotics, pain control, and proper follow-up are essential to prevent complications such as tubo-ovarian abscess, infertility, ectopic pregnancy, and chronic pelvic pain. Prognosis is favorable with timely and adequate treatment.
