Background

Mariya Musa is a three-month-old infant diagnosed with neonatal sepsis chest focus. She is the second child in a family of four. Her mother, Zaikai Yakubu, is a single mother and the primary caregiver of the family. She works as a nanny in a nearby house and earns approximately ₦10,000 monthly. The family resides in a mud house in Zango under difficult living conditions. Mariya is a 3-month-old female infant who was admitted with a history of high-grade fever lasting for four days and vomiting for one day. Developed persistent fever that only reduced temporarily after tepid sponging and administration of paracetamol, associated with brief episodes of focal convulsions involving twitching of the face and hand. On examination, the baby appeared ill-looking, lethargic, febrile, and had difficulty with breathing. Chest examination revealed reduced air entry and bibasal crepitations suggestive of chest infection. She was diagnosed with Neonatal Sepsis with chest focus.

Medical history

The patient was admitted and immediately placed on close monitoring due to the severity of her condition. Initial treatment included administration of intravenous antibiotics, antipyretics for fever control, oxygen support, and intravenous fluids to maintain hydration and stabilize her condition. The child was also managed for the episodes of convulsions and monitored closely for respiratory distress and other possible complications. Supportive nursing care, regular temperature checks, and monitoring of oxygen saturation and feeding tolerance were carried out throughout admission. Laboratory investigations, including Random Blood Sugar, were performed as part of the evaluation and management process. Following commencement of treatment, the child showed gradual clinical improvement with reduction in fever, improved activity level, and better breathing pattern. Vomiting reduced, and no further convulsive episodes were observed during monitoring. The patient remains under medical observation and is responding positively to treatment.

Prognosis

Mariya is a 3-month-old female infant diagnosed with neonatal sepsis with chest focus, presenting with persistent fever, lethargy, vomiting, respiratory distress, and brief focal convulsions. The child needs urgent admission and medical treatment otherwise the young body would be overcome by sepsis and severe complications can set in and can lead to death. Prognosis is good with early intervention, adequate treatment, and proper nursing care, if untreated can have dangerous complications and even death.
