Why is treatment done earlier than admission?

Sometimes, when a case is urgent and vital, hospitals may proceed with delivering medical care right away to save a life, even before the finalization of admission procedures. Usually, when this happens, hospitals put the bill on our credit until the funds are transferred to their accounts.
That's why the chronology can sometimes be in disorder.

Background

Asher Umar is an eight year old girl weighing 20.5 kilograms from a low resource settlement with limited access to clean water and healthcare. She lives with her parents, who are subsistence farmers, in a malaria endemic region. She is an active child who enjoys playing outdoors with other children. Five days before admission, around April 29th 2026, Asher fell onto her outstretched right leg while playing outside and sustained an open wound over the outer side of her right thigh. Over the next 48 hours, the wound became increasingly red and swollen and began draining pus. Asher then developed a high fever accompanied by chills, vomiting, and refusal to eat. Her mother treated her with paracetamol and local herbs, but her condition worsened. She became lethargic, the swelling in her right lower limb extended from the thigh to the knee, and she cried uncontrollably with any movement of the leg. Upon presentation at the hospital, he was evaluated and diagnosed with Acute Hematogenous Osteomyelitis of the right femur and Malaria.

Medical history

Asher has not yet undergone surgical treatment, but she has already received the necessary medical care including intravenous antibiotics and supportive management. She presents with a severe soft tissue infection of the right lower limb, with swelling, redness, pus discharge, fever, and systemic illness. Her condition still requires urgent surgical debridement to remove infected and dead tissue and fully control the source of infection. With surgery and continued care, she is expected to recover, preserve her limb, and regain full function. Thanks to Helpster Charity, hopefuly Asher will walk again after the surgery

Prognosis

Dr. Zainab M. Ngubdo, the attending physician, noticed that Asher presents with a rapidly progressing soft tissue infection of the right lower limb following contamination of an open wound with river water five days ago. She now has spreading redness, swelling, pus discharge, fever, chills, vomiting, poor oral intake, lethargy, and severe pain, indicating a limb- and life-threatening infection. This requires urgent hospitalization, intravenous broad-spectrum antibiotics, surgical debridement, and close monitoring for sepsis. Prognosis depends on early intervention; with prompt treatment, limb function and survival are possible, but delay may lead to sepsis, amputation, or death. Without prompt intravenous antibiotics and surgical debridement, the infection can spread to the bloodstream (sepsis), causing septic shock, multi-organ failure, and death. Limb loss is a real risk if the infection cannot be controlled.
