Background

Conslate is the sixth-born in a family of nine children, all of whom are alive. Due to financial hardship, the first five siblings dropped out of school, while the younger ones continue with their education. She lost her father in 2009, and since then the family has relied on their mother, a peasant farmer, as the sole provider. They live in a small two-room mud-walled house with poor living conditions, and due to limited space, two of her brothers sleep at a neighbor’s home. Despite these challenges, Conslate remains hardworking, hopeful, and aspires to become a teacher in future. Conslate reported symptoms of generalized weakness, nausea, dizziness, headaches, and missed menses, which she initially did not associate with pregnancy. She sought medical review where investigations confirmed pregnancy and she was advised to begin antenatal care (ANC) visits. Out of fear, she initially delayed clinic attendance and kept the pregnancy secret. She later attended ANC regularly from the fourth month onward. She later presented in labour at Ashburn Hospital but, due to non-reassuring fetal status, was referred to Siaya County Referral Hospital for emergency caesarean section.

Medical history

Conslate was taken for an emergency caesarean surgery, the surgery was a success and she was taken to the post -op ward. While in the ward she was administered ceftriaxone,flagyl, morphine, among other drugs. Her catheter was removed after 2 days, and ambulation encouraged. The dressing on her surgical site was removed after 72 hrs. She was able to respond well to medication, and she was eventually discharged on orals. A follow up clinic was booked for her to ensure she was healing well.

Outcome

Conslate presents with preterm drainage of liquor and signs of fetal distress, indicating a high-risk obstetric emergency. She is currently under close monitoring, with efforts focused on stabilizing her condition and preparing for urgent intervention. The fetal distress suggests that the baby may not be receiving adequate oxygen, while the leakage of amniotic fluid increases the risk of infection and preterm complications. At this stage, the prognosis is guarded, as both maternal and fetal outcomes depend on timely intervention. With prompt and appropriate management, there is a good chance of improving outcomes and ensuring the safety of both Conslate and her unborn child.
