Background

Abdulmalik is a 19yo male child who is staying with with his parents 5 of his siblings. The father sells vegetables where he earns little to take care of the family, even at that some of the children cannot go to school. They have 2 rooms in the apartment and a pit toilet. They have a well and no access to electricity. Monthly income is 15000.

Medical history

Abdulmalik is a 19yo SCD patient who presented with history of severe pain on his arms, forearms, thighs and legs of 6 days duration pain keeps waxing and waning. He has not been able to walk due to the pain. There is a preceding history of exposure to mosquito bites and fever. No abdominal pain and no respiratory problems. The last time he had sickle cell crisis was about a year ago. He is not sure of his steady PCV however he attested to having PCV of between 20 and 30 whenever it is checked during crisis. He was diagnosed with Sickle Cell Long Bones Crisis. Patiently was immediately admitted and commenced treatment with some painkillers as well as antibiotics, later on patient was transfused 1 pack of red blood cell. Abdulmalik recovered well, feeling of pains stopped as well as fever. Patient was discharged with some orals for continued care.

Prognosis

Abdulmalik presented with Sickle cell long bone crisis which is a serious but manageable complication requiring prompt pain control, hydration, oxygen support, and treatment of triggers like infection. Early medical intervention reduces complications and improves recovery. A sickle cell bone crisis can lead to acute complications like bone infarction (tissue death due to a lack of blood supply), osteomyelitis (bone infection), and permanent joint destruction or avascular necrosis. He requires an urgent intervention. PROGNOSIS The long-term prognosis of sickle cell disease is significantly improved by modern therapies like hydroxyurea and gene therapy, though chronic bone crises steadily increase the risk of permanent joint disability and reduced overall life expectancy. Patient will recover well with proper care, though recurrent crises may cause chronic pain, bone infarction, infection, or joint damage. Long-term outcome depends on crisis frequency, treatment adherence, and overall disease management.
