Sometimes, when a case is urgent and vital, hospitals may proceed with delivering medical care right away to save a life, even before the finalization of admission procedures. Usually, when this happens, hospitals put the bill on our credit until the funds are transferred to their accounts.
That's why the chronology can sometimes be in disorder.

Background

Baraka Nasir is a 17-month-old baby girl who lives with her widowed mother and three older siblings in a small mud house near Birniwa. Her father died before she was born, and the family was displaced by insurgency, forcing them to live for years in an IDP camp. Her mother survives by selling firewood and washing clothes for neighbors, earning barely enough to feed her children. A few weeks after being treated for an upper respiratory tract infection, Baraka became ill again with high fever, persistent cough, difficulty breathing, poor feeding, and weakness. Her worried mother watched helplessly as her daughter grew weaker and struggled to breathe, but she could not afford medical care. Hoping for help, she brought Baraka to Adari Medical Clinic through the Helpster Charity Foundation. After examination and investigations, Baraka was diagnosed with typhoid fever and a relapsed respiratory tract infection complicated by bronchopneumonia. Because she was vomiting, dehydrated, and in respiratory distress, she was admitted for urgent treatment with intravenous fluids, antibiotics, and oxygen therapy. After stabilizing and showing improvement, she was discharged on oral medications, giving her family renewed hope for her recovery.

Medical history

Baraka, a 17-month-old girl from a displaced family, was brought to Adari Medical Clinic with worsening difficulty in breathing, fever, and poor feeding despite previous treatment for cough. She was admitted and managed with oxygen therapy, intravenous fluids, intravenous ceftriaxone, and supportive care while her respiratory status and oxygen saturation were closely monitored. Within the first 12 hours of treatment, her breathing improved, her oxygen levels stabilized, and her fever began to subside. Baraka responded well to treatment and showed encouraging signs of recovery. Prognosis

Baraka Nasir, presented with a three-day history of high fever, cough, difficulty breathing, poor feeding, vomiting, and lethargy. She had been treated for URTI two weeks ago but relapsed with worsening severity. On examination, she was febrile (39.2°C), pale, lethargic, dehydrated, with nasal flaring, subcostal retractions, crackles on lung auscultation, and oxygen saturation of 92% on room air. Laboratory tests confirmed acute typhoid fever and elevated inflammatory markers consistent with bacterial respiratory infection. She was diagnosed with typhoid fever with respiratory tract infection relapse (bronchopneumonia). This is a serious, potentially life-threatening condition requiring urgent medical treatment to prevent complications such as respiratory failure, sepsis, and death. Because she was unable to take oral medications due to vomiting and had respiratory distress, she was admitted for 1 day for intravenous fluids, intravenous antibiotics, and oxygen therapy. She received intravenous ceftriaxone, intravenous fluids for rehydration, intravenous paracetamol for fever, and supplemental oxygen via nasal cannula. She responded well to treatment. Her oxygen saturation improved to 95%, hydration status improved, and she began feeding orally. After 24 hours, she was clinically stable and discharged with oral antibiotics. Prognosis is excellent with prompt treatment. Full recovery is expected within 1-2 weeks. If treatment had been delayed, she would have risked severe pneumonia, respiratory failure, sepsis, and death.
