Why is treatment done earlier than admission?
Sometimes, when a case is urgent and vital, hospitals may proceed with delivering medical care right away to save a life, even before the finalization of admission procedures. Usually, when this happens, hospitals put the bill on our credit until the funds are transferred to their accounts.
That's why the chronology can sometimes be in disorder.

Background
Jibrin Mohammed is a 2-year-old boy living with his parents and three siblings in a low-income household in Birniwa town. His father works as a laborer while his mother is a homemaker. Unable to afford medical care, the family sought assistance through the Helpster Charity Foundation when Jibrin became seriously ill. He presented with a 5-day history of high fever, chills, vomiting, abdominal pain, poor appetite, and progressive weakness. Despite home treatment with herbal remedies, his condition worsened, and he became lethargic and refused to eat or drink. His mother also reported that he had passed worms in his stool on several occasions. On examination, Jibrin was critically ill with a temperature of 40.1°C, pallor, lethargy, abdominal tenderness, mild hepatosplenomegaly, and dehydration. Laboratory investigations confirmed severe Plasmodium falciparum malaria with high parasitemia (5.8%), severe anemia (Hb 7.2 g/dL), and heavy Ascaris lumbricoides infestation on stool microscopy. He was diagnosed with severe malaria complicated by severe anemia and intestinal helminthiasis and was admitted for intravenous antimalarial therapy, blood transfusion, deworming treatment, intravenous fluids, and close monitoring.
Medical history
Jibrin, a two-year-old boy, was brought to Adari Medical Clinic with severe malaria and heavy worm infestation. Following clinical evaluation and laboratory confirmation, he was admitted and treated with intravenous antimalarials, blood transfusion, deworming medication, and supportive care. His parents were educated on the importance of hygiene, sanitation, and malaria prevention. Over the course of his admission, Jibrin showed gradual improvement his fever resolved, his appetite returned, and he regained his strength. He was discharged home in stable condition with instructions for follow-up. We thank Helpster Charity Foundation for providing the support that made Jibrin's treatment and recovery possible.
Prognosis
Jibrin Mohammed presented with severe malaria complicated by severe anemia and heavy intestinal parasitic infestation, a combination that constituted a life-threatening medical emergency requiring urgent inpatient management. His condition was appropriately managed with intravenous antimalarial therapy, blood transfusion, deworming treatment, supportive care, and close clinical monitoring. He demonstrated an excellent clinical response, with resolution of fever, clearance of parasitemia, improvement in hemoglobin levels, and successful treatment of the intestinal parasitic infection. The prognosis is good. Given his favorable response to treatment and absence of ongoing complications at discharge, he is expected to make a full recovery without long-term health consequences. Continued nutritional support, routine follow-up, and measures to prevent future malaria and parasitic infections are recommended to maintain his recovery.
