Why is treatment done earlier than admission?
Sometimes, when a case is urgent and vital, hospitals may proceed with delivering medical care right away to save a life, even before the finalization of admission procedures. Usually, when this happens, hospitals put the bill on our credit until the funds are transferred to their accounts.
That's why the chronology can sometimes be in disorder.

Background
Usman Abubakar is an eleven-year-old boy with a quiet determination and a resilient spirit. He lives with his parents and sevenr younger siblings in a small mud house in a poor settlement in sabon layi of Birniwa town. His father is a former taxi driver with no earnings now. His mother is a janitor at secondary school who cares for the children and does what she can to stretch the little money they have. They have no savings, no health insurance, and no family support. Usman is the eldest child and often helps his father with farm chores after school. Despite the hardship, his parents dream of him growing up healthy, completing his education, and having a better future than their own. Five days before coming to the hospital, Usman fell while playing outside and sustained a forearm injury. His parents, unable to afford medical care, treated him with local herbs and bandaging. When the pain and swelling worsened and he developed a high fever, they finally brought him to Adari Medical Clinic on 18th June 2026, seeking help for their son. He was diagnosed with both-bone forearm greenstick fractures with mild angulation, complicated by malaria and hypocalcemia.
Medical history
Usman, an eleven-year-old boy, was brought to Adari Medical Clinic with a fractured forearm that had been untreated for five days, complicated by malaria and low calcium levels. Following clinical evaluation, X-ray, and laboratory confirmation, he was admitted for closed reduction and immobilization, antimalarial treatment, and calcium supplementation. He received pain management and close monitoring. Over six days, Usman showed gradual improvement his fever resolved, his calcium levels normalized, his pain decreased, and swelling reduced. He was discharged home with the cast in place and instructions for follow-up. We thank Helpster Charity Foundation for providing the support that made Usman's treatment and recovery possible.
Prognosis
Usman Abubakar, an eleven-year-old boy, presented with a right forearm fracture sustained five days prior, complicated by malaria and hypocalcemia. He had high fever (39.2°C), pain, swelling, and deformity of the right forearm. X-ray confirmed both-bone greenstick fractures of the midshaft radius and ulna with mild angulation. Laboratory tests confirmed malaria (2.4% parasitemia), hypocalcemia (serum calcium 7.2 mg/dL), and elevated inflammatory markers. He was diagnosed with both-bone forearm greenstick fractures with mild angulation, complicated by malaria and hypocalcemia. He needed closed reduction and immobilization with an above-elbow cast, antimalarial treatment, calcium supplementation, and close monitoring. He was admitted from 18th to 24th June 2026 (6 days). He underwent closed reduction under sedation, followed by application of an above-elbow cast. He received intravenous artesunate, calcium supplementation, and pain management. He responded well to treatment, pain decreased, swelling reduced, fever resolved, and calcium levels normalized. At discharge, he was stable with the cast in place and instructions for follow-up. Prognosis is excellent with proper treatment. Full recovery expected within 4-6 weeks.
