Joyce A., 15

Joyce is the second-born in a family of five children. She is currently in Grade Seven, while her siblings are also in school except for the youngest, who dropped out because of financial constraints. Joyce's parents separated three years ago, and the family lives in a modest two-room mud-walled house with an iron-sheet roof. Their limited financial resources make it difficult to meet the family's basic needs. Joyce developed progressive body weakness, excessive sleepiness, persistent yellowish diarrhoea, repeated vomiting, and poor oral intake. Despite receiving medication from a nearby pharmacy, her condition continued to deteriorate. Joyce is a known patient with rheumatic heart disease, and the severe diarrhoea and vomiting had caused significant dehydration. In patients with heart disease, dehydration and fluid replacement must be managed very carefully, as both inadequate and excessive fluids can place dangerous strain on the heart. She therefore required inpatient admission to stabilize her condition and prevent potentially life-threatening complications.

Joyce came to the hospital with complaints of general body malaise, hotness of the body, abdominal pains and passion of looe stool .she was diagnosed with acute gastritis, mild anemia,rheumatic heart disease and congestive heart failure. This prompted admission where an EcG was done, she was also administered morphine, omeprazile, JASA 300mg Stat, lasix among other drugs to which she was tolerant. Her vitals were constantly monitored and chartered, with repeat ECG done. She eventually responded to treatment and was discharged on orals and advised to seek further medical evaluation at higher level facility.

Joyce a 15-year-old patient has rheumatic heart disease complicated by congestive heart failure, requiring close monitoring and long-term medical management. Acute gastritis and mild microcytic anemia may worsen weakness and poor appetite, with timely and appropriate treatment With appropriate treatment and regular follow-up, the condition can be stabilized, this makes her to have a guarded but potentially favorable prognosis although the underlying heart valve damage is permanent and requires lifelong care.
