Why is treatment done earlier than admission?

Sometimes, when a case is urgent and vital, hospitals may proceed with delivering medical care right away to save a life, even before the finalization of admission procedures. Usually, when this happens, hospitals put the bill on our credit until the funds are transferred to their accounts.

That's why the chronology can sometimes be in disorder.

Background

Dexter is a 15-year-old boy and the fourth-born in a family of five children. His father supports the family through small-scale farming, while his mother is a homemaker. Despite their efforts, the household’s limited income is often insufficient to meet daily needs and unexpected medical expenses. Dexter is polite, hardworking, and determined. He enjoys football, reading, and participating in school activities. He hopes to become an engineer and believes that education and perseverance will enable him to support his family and build a better future. Dexter is living with sickle cell disease and had been managing well until he suddenly developed severe chest pain, joint pain, and a persistent cough. Concerned about the rapid change in his condition, his father took him to Bokoli Sub-County Hospital. He chose the facility after learning through a community outreach that Helpster Charity assists vulnerable children with medical expenses. Following clinical assessment and laboratory investigations, Dexter was diagnosed with pneumonia alongside sickle cell disease crisis. Due to the seriousness of his condition, he was admitted.

Prognosis

Dexter was seriously ill with severe pneumonia complicated by a sickle cell crisis. The lung infection could reduce his oxygen levels and worsen sickling, increasing the risk of acute chest syndrome, severe anaemia, respiratory failure, or organ damage. Admission was necessary for antibiotics, oxygen when indicated, pain control, hydration, monitoring, and supportive care. His immediate prognosis is favourable with prompt treatment. Long term, he requires regular sickle cell clinic follow-up, prescribed medication, vaccination, adequate hydration, and urgent review whenever he develops fever, chest pain, cough, or breathing difficulty.
