Why is treatment done earlier than admission?

Sometimes, when a case is urgent and vital, hospitals may proceed with delivering medical care right away to save a life, even before the finalization of admission procedures. Usually, when this happens, hospitals put the bill on our credit until the funds are transferred to their accounts.

That's why the chronology can sometimes be in disorder.

Background

The 30-year-old pregnant woman is a hardworking homemaker who enjoys making nakshi and dreams of building a permanent home for her family while hoping for a safe delivery and a healthy baby. She lives with her husband, their 11-year-old daughter, and her elderly in-laws in fragile housing conditions, while her husband works as a day labourer earning only approximately USD 49 per month, making it extremely difficult to cover basic necessities and costly maternity care. At 39 weeks and 4 days of pregnancy, she developed lower abdominal pain, decreased fetal movement, abdominal tightening, and watery vaginal discharge, prompting her to seek medical care at New Evercare Hospital. Following clinical assessment and ultrasonography, doctors diagnosed breech presentation with severe oligohydramnios, requiring close monitoring and appropriate obstetric management to protect both mother and baby. Without timely medical intervention, the condition could lead to fetal distress, umbilical cord compression, oxygen deprivation, and potentially life-threatening complications for both mother and baby.

Prognosis

The recommended treatment is a Lower Uterine Cesarean Section (LUCS) under spinal anesthesia (SAB), considering the patient's breech presentation and severe oligohydramnios. Timely surgical delivery, along with appropriate medical management and close monitoring, may reduce the risk of complications and improve the chances of a safer outcome for both mother and baby. If delivery is delayed, the baby may be at increased risk of fetal distress, umbilical cord prolapse or compression, oxygen deprivation, birth trauma, and head entrapment. The mother may also face serious delivery-related complications, highlighting the importance of timely obstetric intervention.
