Background

Imeh Patience is a 26-year-old pregnant mother of four whose family faces extreme financial hardship and homelessness after her husband lost his job as a ship chef. Following a tragic December 2025 fire outbreak that claimed the life of one of their children, they were evicted and now sleep outdoors. Due to these overwhelming hardships, she was unable to afford or access antenatal care or purchase any items for the baby. The patient is a 33-week pregnant multigravida (G5P4) presenting with 3rd Trimester Cyesis, severe Pregnancy-Induced Hypertension (PIH) with a blood pressure of 168/91 mmHg, and a singleton viable fetus in breech presentation. Upon presentation at our facility, she was evaluated and diagnosed with uncontrolled pregnancy-induced hypertension combined with a persistent breech presentation in an unbooked third-trimester pregnancy. Given her age and multiple pregnancies, she requires urgent medical intervention and is scheduled for a Caesarean Section upon approval.

Prognosis

The 45-year-old Patience is an unbooked multigravida with a high risk of complications due to pregnancy-induced hypertension and breech presentation. She requires urgent medical stabilization and a definitive Caesarean section. This combination significantly increases the risk of eclampsia, placental abruption, maternal stroke, umbilical cord prolapse, head entrapment, and fetal death. With timely delivery and rigorous blood pressure control, the prognosis for both mother and child is excellent. This intervention will directly avert catastrophic complications, ensuring maternal and neonatal survival.
