Why is treatment done earlier than admission?

Sometimes, when a case is urgent and vital, hospitals may proceed with delivering medical care right away to save a life, even before the finalization of admission procedures. Usually, when this happens, hospitals put the bill on our credit until the funds are transferred to their accounts.

Background

He is empathetic and obedient child who always like playing football. He is aspiring to become a professional footballer to play for his country national team (Super eagles) was history of low urine output, increase frequency, passage of stone and blood in urine however, no dysuria or history of fever. Attempt to pass urethral catheter prove abortive but suprapubic tap was done to relieve the patient pain. On examination: Abd- full, MWR, tender suprapubic region, liver and spleen not palpably enlarge, kidneys not ballotable. A diagnosis of acute urinary retention secondary to bladder stone was made.

Medical history

Bulau had undergone a Cystolithotomy under general anesthesia. The surgery was successful, with no surgical site infection or other complications. The patient had been on post-operative parenteral antibiotics - IV Amoxiclav, IV Flagyl in addition to analgesics and intravenous fluid. He later commenced ambulation and graded oral sip, and his medications were converted to orals. As the patient's recovery advanced, he was discharged home and given a follow-up appointment of one week.

Outcome

This condition requires urgent surgical intervention. Plain abdominal X-ray, Urinalysis, EUCr, FBC is essential for complete evaluation of the patient. Cystolithotomy is the definitive treatment for this child. If left untreated, acute urine retention can lead to complications such as: Bladder damage, Recurrent urinary tract infections, Kidney damage, Sepsis.
