Why is treatment done earlier than admission?

Sometimes, when a case is urgent and vital, hospitals may proceed with delivering medical care right away to save a life, even before the finalization of admission procedures. Usually, when this happens, hospitals put the bill on our credit until the funds are transferred to their accounts.
That's why the chronology can sometimes be in disorder.

Background

Elroi Niya is the 2nd child of his parents. He is just a few weeks old. They live in Kaduna State Nigeria. His parent are trying to keep him alive and hoping he survive the surgery and live a normal life like every other kid. The mother had to quit her job and source of income to take care of the sick child. And the father is doing local logistics, his income is temporary and cannot cover bills. The above 7-weeks old child was noted to have developed a swelling at the lower back immediately after birth. The swelling did not disappear and the child was taken to the hospital for evaluation. On presentation at our hospital, the child had a ruptured swelling at the lower back which was leaking cerebrospinal fluid. He was evaluated and a diagnosis of ruptured myelomeningocele was made. he was managed conservatively with antibiotics and daily wound dressing until he is fit for surgery. A week earlier, he had a tensed anterior fontanelle which was confirmed to be Hydrocephalus after the brain CT scan. The child needs urgent surgery for excision and repair of the myelomeningocele and ventriculoperitoneal shunt insertion.

Medical history

Baby Elroi has undergone surgery and is still on admission. He looked better and responding well to treatment. The above named infant with lumbar myelomeningocele and hydrocephalus had Ventriculoperitoneal shunt incision and Excision and repair of the myelomeningocele on 11th April, 2025. He did well and was discharged on 22nd April, 2025. He is currently being seen at our Neurosurgical Outpatient Clinic.

Outcome

A cranial CT scan showed features of hydrocephalus therefore needing two (2) surgeries of Excision and repair of the myelomeningocele and ventriculopertoneal shunt insertion.
