Why is treatment done earlier than admission?

Sometimes, when a case is urgent and vital, hospitals may proceed with delivering medical care right away to save a life, even before the finalization of admission procedures. Usually, when this happens, hospitals put the bill on our credit until the funds are transferred to their accounts.
That's why the chronology can sometimes be in disorder.

Background

Phanice is a 16-year-old girl and a Form Two student at a local public secondary school. She lives with her single mother, who is the family’s sole provider. Her mother works as a casual laborer, earning small and irregular wages that are often insufficient to meet their basic daily needs such as food, rent, and school expenses. As a result, life has been very challenging for them, and they frequently struggle to make ends meet. In an effort to support herself and remain in school despite these difficulties, Phanice unfortunately became pregnant. This situation placed a significant emotional and financial strain on both her and her mother. Despite these challenges, she carried the pregnancy to full term. However, during delivery, she developed serious complications that required an emergency cesarean section to save both her life and that of her baby. The surgical procedure and the subsequent hospital stay resulted in a large medical bill that her mother is unable to pay due to their limited financial resources. Phanice is currently recovering from the operation, but the unpaid hospital bill has added further stress to the family’s already difficult circumstances. Phanice is therefore humbly seeking assistance from Helpster Charity to help clear the outstanding hospital bill and support her recovery. With such support, she hopes to regain her health, care for her newborn child, and eventually return to school to continue her education and build a better future for herself and her baby. Phanice, a primigravida at term according to her antenatal clinic records, presented to the hospital in active labour accompanied by her mother. She complained of severe abdominal pain radiating to her back, which had been increasing in both frequency and intensity and was not relieved by analgesics. She also reported experiencing leakage of fluid for the past two days, which initially occurred in small amounts but had increased significantly on the day of admission. Following a thorough clinical examination, she was diagnosed with malpresentation (breech presentation) accompanied by fetal distress. Due to these complications, the medical team recommended an emergency cesarean section as the safest and most appropriate intervention to protect the lives of both the mother and the baby.

Medical history

Phanice brought to the facility in labour by her mother with complains of severe abdominal pain radiating to the back increasing in frequency and intensity not responding to analgesic, she reports that she has been draining liquid for the last two days in small amount but increased in amount the following day. Following clinical examination, laboratory investigation and ultrasound, she was at 3 cervical dilation and Meconium grade 2 was noticed, based on findings she was diagnosed with malpresantation with fetal distress. Due to this complications doctors made an urgent decision to perform emergency ceaserian section for the purpose of saving lives of mother and the baby, Operation was done successfully both mother and the baby were in good state and stabilized after awhile and discharged on good condition.

Outcome

Clinical assessment revealed that the fetus was in breech presentation, a form of malpresentation where the baby’s buttocks or feet present first instead of the head. In addition, there were signs of fetal distress, indicating that the baby was not tolerating labor well and could be at risk of complications if delivery was not expedited. Given these findings, the situation was considered an obstetric emergency. Vaginal delivery in the presence of breech presentation and fetal distress could significantly increase the risk of complications for both the mother and the baby, including birth trauma, prolonged labor, or compromised oxygen supply to the baby. Following emergency cesarean section and appropriate postoperative care, Phanice’s prognosis will be generally favorable. With proper wound care, adequate rest, and adherence to medical advice, she is expected to recover well from the surgical procedure.
