Princess N., 4

Princess is a four-year-old girl and the only child of her mother. She lives with her mother in her aunt’s home, where they share a two-roomed, mud-walled house with five cousins. Both her mother and aunt are peasant farmers with no stable income, and the family faces significant financial challenges. When well, Princess is a cheerful and playful child who enjoys interacting with others. According to her mother, Princess had been healthy and active until the day she suddenly developed nosebleeds. Initially, her mother thought it was a minor issue that would resolve on its own. However, soon after the bleeding began, Princess became weak, lost her appetite, and refused to eat. Due to limited resources, her mother attempted to manage the condition at home by giving her paracetamol, hoping she would improve. Unfortunately, Princess’ condition worsened, with the nosebleeds persisting and her weakness becoming more pronounced. Recognizing that the situation was serious, her mother sought medical care at a nearby health centre. Upon assessment, the healthcare providers noted the severity of her condition and referred her to Siaya County Referral Hospital for further evaluation and specialized treatment.

Princes was brought to the hospital with complaints of nose bleeds, lack of appetite and body malaise. She was diagnosed with severe anemia, severe malaria and thrombocytopenia. She was promptly admitted to the ward where she was transfused blood, administered IV Artesunate, pcm , omeprazole among other drugs. She responded well to medication with and eventually her bleeding stopped, her haemoglobin became stable and malaria resolved. She was eventually discharged on orals and a follow up clinic booked for her. A call to her mother revealed that princess was well and could play as before.

The prognosis for a 4-year-old diagnosed with severe anemia, severe malaria and thrombocytopenia is grave and represents a critical medical emergency. While medical science has improved survival, this specific trio of complications is highly fatal if not managed with immediate intensive care. Prognosis is fair
