Irene S., 4

Irene is a cheerful and resilient 4-year-old who lives with her mother and five siblings in a two-room mud house with an unfinished kitchen. The family uses a pit latrine and walks about 20 minutes to fetch water from a stream. Her mother, the sole breadwinner, works as a school cleaner earning approximately KSh 2,000 monthly, leaving the family with very limited resources. A friend had given the family an organophosphate-containing insecticide to control a persistent bedbug problem. It was stored in a soda bottle and accidentally left within Irene’s reach. While her mother was tending the kitchen garden, Irene was found with the bottle empty, raising concern that she had ingested the chemical. Initially she appeared well, but about four hours later developed abdominal discomfort, vomiting, diarrhoea, chest discomfort, difficulty breathing, headache and poor appetite. Her mother initially gave her paracetamol, but her condition worsened. Despite having no money for treatment, she rushed Irene to hospital, where she learnt about Helpster Charity. Irene was enrolled for support, urgently assessed, admitted and placed under close medical monitoring and treatment.

Irene was admitted to the hospital after being diagnosed with Organophosphate Poisoning and Aspiration Pneumonia. During her 3-day stay, she received prompt medical care including antidotes to counter the poisoning, oxygen therapy to support her breathing, and antibiotics to treat the lung infection. She was closely monitored by healthcare providers to ensure her condition stabilized and improved steadily. Following treatment, Irene responded well and recovered fully, with her symptoms resolving and her strength returning. She was safely discharged in good health. Her mother expressed heartfelt appreciation to Helpster Charity Organization and Lumakanda Hospital for their timely support and compassionate care, which played a crucial role in saving Irene’s life.

Irene’s presentation is consistent with acute organophosphate poisoning following suspected accidental ingestion, with gastrointestinal and respiratory symptoms requiring urgent hospital management and close monitoring. At her young age, significant exposure can cause serious complications, particularly breathing difficulties and neurological effects. However, with prompt treatment, supportive care and careful monitoring, her prognosis is favourable if she continues to respond well. Continued observation is important until she is clinically stable and free from complications.
